Have you gotten a notice from PUA about uploading proof that you lost work during the pandemic?
If you applied for PUA in 2020 and are still collecting in 2021, you just need a single document that shows you were working at some point between January 2019 and your application for PUA. It could be pay stubs, tax documents, contracts, business licenses, letters, etc. If you don’t have any documents uploaded to your portal, you have 90 days from the day you got the notice to upload a document, and that deadline can be extended with good cause. If you already uploaded documentation, don’t stress, you don’t need to do more.
If you don’t have this type of documentation, like many people who work ‘under the table,’ you can fill out this form explaining the type of work you did and contact info for your employer, and then upload it to your PUA portal. Whenever possible, you should upload a document that proves you worked instead of using this form. This form may not satisfy PUA’s legal requirement for documentation of work.
